For new patients taking advantage of our complimentary LASIK evaluation, we ask that you complete the following LASIK Information Form. Please fill out the form, save and print the form, then bring the completed form to our office. This will allow us to get started with your VIP LASIK evaluation as soon as you arrive.
For patients who have scheduled for a Cataract Evaluation, we ask that you complete the following Cataract Questionnaire below. This form helps the surgeon determine the best surgical plan for you. Please fill out the form, save and print the form, then bring the completed form to our office.
